My Forms

Please fill out your new patient forms ahead of time and print them out to bring to your first appointment. This will allow us to spend your valuable time getting to know you and your dental goals!

We accept cash, check, Visa, MasterCard, Discover, American Express, and CareCredit®. We also accept most major insurances. If any of these options aren’t for you, we provide an in-house Healthy Smiles Savings Plan as well.
